Q-State’s Privacy Policy summarizes what personally identifiable information we may collect, and how we might use this information. Information You Provide

Q-State collects information you knowingly and voluntarily provide to us, such as information you provide when signing up to receive e-mail alerts, completing a survey, making data entries in connection with the use of any web-based Q-State services to you, or when asking us a question or providing feedback. This information may include, personally identifiable information (i.e., your name, telephone number, e-mail, or health or medical information). You will be informed what information is required and what information is optional.

Q-State is committed to protecting the privacy of children. We do not knowingly collect personally identifiable information from a child under the age of 13.

Information Collected Passively

When you visit the Q-State website, certain anonymous information may be passively collected through the use of Internet tags, or other navigational data collection including log files, server logs, or clickstream. California Do Not Track Disclosure

Q-State does not track its customers over time and across third party websites to provide targeted advertising and therefore does not respond to Do Not Track (DNT) signals. Q-State uses personally identifiable information only for the purposes for which you provide it and to communicate information to you (if requested), except that Q-State may use information entered into the Q-State Medication Diary to conduct research, development and project planning after any personal details from the information is redacted.
